You can now use our online scheduling tool from your device to request an appointment. The tool allows you to view currently available appointment times and submit an appointment request.

All appointment requests are reviewed by our Intake Coordinator within one business day. Please note that submitting a request does not guarantee an appointment or confirm the requested time.

After the Intake Coordinator reviews your request, you will receive any required paperwork and instructions for submitting the necessary uploads. Your appointment will not be considered confirmed until all required paperwork has been completed and all required uploads have been submitted.
